Adrenal Fatigue Isn't Real. Here's What Actually Happens Instead.
I hear it most weeks. A client sits down, exhausted, and tells me their doctor said their adrenals were fine, but they know something's wrong, because they've read about adrenal fatigue and it sounds exactly like them.
Here's the thing. Their doctor is right, and so are they, just not quite in the way either of them thinks.
Adrenal fatigue, as it's usually described, tired glands that have run out of cortisol to give, isn't a recognised medical condition. True adrenal insufficiency is Addison's disease, a serious condition that needs proper medical management. That's not what's happening in most of the exhausted, wired but tired people I see.
What's happening is more interesting, and more useful to understand, because it points to something you can work with. It isn't your adrenal glands failing. It's your whole stress response system adapting to prolonged demand.
When cortisol stays elevated for a long time, the receptors that respond to it become less sensitive, a bit like turning the volume down on a sound you've heard too long. That blunts the feedback loop meant to bring cortisol back down, so you end up with a system that's both overexposed and under supervised. Structural changes happen too, in the brain regions that regulate this whole process, and in the glands themselves, the balance between the different parts of your adrenal glands can shift. Your pituitary gland can shrink slightly, essentially protecting itself from constantly being asked to respond to relentless cortisol signalling.
This is why a saliva cortisol test can come back looking normal in someone who is clearly struggling. The test isn't wrong. It's measuring a system that has already adapted to the demand it's been under. Normal on paper doesn't always mean settled underneath.
From a Chinese medicine view, this pattern often shows up as depleted Kidney Qi, the reserve energy we're each born with and draw on under sustained pressure. It's a helpful way to think about it, because it captures something the Western model can miss, that this is about reserves being spent faster than they're replenished, not one organ breaking down.
The good news is that adaptation can go the other way too. This isn't gland failure, it's a system that has learned to run in a certain state and can, with the right support, learn a different one.
